I didn't see it as an issue, Dawn would just tease her about it, and I found it funny. It was also ridiculous that Buffy made Dawn hide in the kid casket when she is clearly taller than her haha the writer's also probably liked poking fun at how tiny Sarah is.
I'm assuming you're referring to this scene in Normal Again when Buffy thinks she's at the mental hospital:
I think the point of this was to show that Buffy wasn't thinking clearly. Since older siblings are typically taller than younger siblings until they're more or less fully grown, it seemed odd to Buffy that Dawn was taller than her because she was unable to recognize that Dawn was old enough to be taller than her due to being poisoned by that demon. Instead, Buffy used their height difference as a reason why Sunnydale couldn't be real. To her poison-addled brain, it seemed inconsistant and one of the many reasons she had to be imagining her life as a Slayer.

My sister's 7 years older than I am, and I've got a good 4 inches in height on her. Which is not something I relish at all.

I didn't think it was an issue. I can see why Buffy would be annoyed by it though. I don't have any siblings but my cousin who is 3 years younger is taller than me and it annoys me 'cause in my head he's supposed to be just a baby but he's massive 😛 The others are well on their way to being taller than me too. It's not fair! It's hard to maintain your eldest authority when the younger is glaring down at you!

Often when a sibling is younger and taller at the same time, it is often a teasing thing. I am the eldest at 16 years but I was done growing by 10. I'm about 5'2. My sister is 14 and she is now taller than me and is still growing. She teases me all the time.
I tease my sister all the time because she is waaay shorter than me haha i even tease my mom because she is even shorter than both of us! I love that i am the youngest and have been teased my whole life by my siblings but now i can at least look down on them physically 😛. Even my older brother isnt any taller than me now, and he gets teased all the time about his little sister catching up in height 🙂
