The war on demons is like any other war - sometimes the good die and others come through it without a scratch.
In the commentary episodes in the box set, Joss mentioned that Charisma had to get approval for the haircut in Season 3. This would be a standard clause in any actor's employment contract - any appearance changes require notification and a possible veto from the producer. I can understand that there was a past history of not being able to carry a pregnancy to term, but this is not an excuse for not notifying the producer.
I used to (among other tasks) write sections of business proposals and it is a process where you are on duty 60 hours a week until your section is done. In other commentaries, Joss said that he had a staff of 237 people working for him and between seasons in the summer, his writers try to get a head start on the new season and his set designers, schedulers, location scouts and many of the other staffers have to work similarly long hours getting the new season planned out. Now imagine that you have to scrap several month's effort from dozens of these high-priced people because one of the prime actors was going to change in appearance and become unavailable at some point but did not mention it when there was a chance to redirect the screenwriters' efforts. The time lost was even more important than the money - money can be replaced.
I think Joss would have been able to sue for breach of contract and if he didn't, it is because he is the good guy in this story. I think Charisma has to bear some of the responsibility for the lameness of seasons 4 and 5. Her response in one interview that the result of her only having a token appearance in Season 5 was that the show got cancelled is probably true - but she was partly responsible.
