According to Willow in the IDW Spike miniseries, and what we learned from the last couple of issues of Season 8, Buffy had known about Spike being back for at least several months, and from what Willow told him about the "kicking down doors" part, it seems that no one was running happily up to her proclaiming "Spike's alive! Spike's alive!"
So, just on conjecture, what first raised Buffy's suspicions about Spike being back, and whose door do you think got kicked in?
My guess would be that Giles first found out, probably from Wes, who would have almost certainly have figured that Spike's return was information of note, and something that the Watchers and Slayers should know. (People had "come back wrong" before, primarily Cordy in Season 4 and Spike himself in Season 7 of "Buffy," and, arguably, Connor. If it were happening again, somebody should know and be preparing for the consequences) If that were the case, Giles might have decided to keep the knowledge to himself, and keep a close eye (or group of eyes, the 4 Slayers in the "After The Fall" arc of Angel's book) Buffy would have found about the Slayers, gotten her suspicions aroused, figured that SOMETHING was going on, and decided to find out exactly what.
I don't think that the logic of "Way too many people die around the hellmouth, and more than a few of them don't stay that way," escaped Buffy either.
