Requesting a Banner

I'd really appreciate it if someone would be kind enough to make me a banner using these pictures and with the text "Invariably Fatal". It doesn't have to be all of the pics or anything, I just thought I'd give the maker a choice and they could use what they wanted/needed 🙂
Oh and could the banner be not bigger than 350x150 please?
