Upon reflection, Illyria. Joss basically took the feelings of helplessness one has when a loved one has a unexpected terminal disease, and turned it into an monster. Most of us aren't being stalked at night by a lion, but all of us will know that fear. To add worst, the monster kills her victim, and wears her corpse around as a trophy, meaning, you don't get to stop grieving. I mean, Vampires have been doing that since the start of the series, sure, but it seems more vile in her body snatching way. Plus, he did it with a girl. Imagine if Geena Davis became the Fly, and Seth spent the film trying to save her, only having to kill her in the end. That's what Illyria basically was. Joss nailed it.
 
In terms of gruesomeness, Gnarl, no question. Though from a viewpoint of how dangerous they are, I don't think anything was less slayable than Dracula. I don't think Joss implemented the myths of his powers well because he seems essentially unkillable and yet has no desire to commit actual evil.
 
I agree with @Glory Worshippe in that the qwellor is the only monsterish looking monster to make my skin crawl. Other than this, I mostly get the creeps from types like Ted, Warren, and Caleb, because I find them all too real. There is an element of "oh this could happen" with those characters. Caleb, for one, reminds me of my former best friend's husband, except that he's way better looking. Ted was really scary, though! I got anxiety just watching that episode.
