I think it's sad if done just for looks (unless it's like recovering from burns and such), but I'm not typical.
Going strictly by my personal experience, I'd say most Americans don't think about it that much one way or another, but those I talk to also tend to be a bit different from the mainstream as well. I do share small talk with plenty of casual acquaintances, and it never came up. I do recall overhearing some men at a gym I used to go to complaining about beauty standards in women being ridiculous, and prefer women with curves rather than looking like china dolls or little boys (or words to that effect, it was years ago). They were talking about the fashion industry and Hollywood rather than cosmetic surgery specifically, but I believe one said something about "having boobs too big for a body that thin" or some such which probably refers to cosmetic surgery.
Shows from Hollywood show it differently, and it would make sense that it would be seen in a different light there than elsewhere (which gets reflected in their stories). I know I've seen some news stories about it (getting the surgeries for teens is what they usually focus on, and an archived example can be found
here), and I heard it was a hot topic on some woman's talk show, and my impression is that for the most part people frown on it enough while being tempted (I'm sure they advertise through a variety of means) that it can gain and audience (thus advertising dollars), but not such a hot topic that there is that much on it. But I want to emphasize the focus is on parents (usually mothers who get plenty of these surgeries for themselves) who pay for their daughters to have them, and not on celebrities.
As for celebrities...they are generally excepted from the standards of the rest of us, so different standards apply to them. I think many if not most just want them to entertain us and thus get a lot more leeway. But I avoid most gossip mags and shows so I may have simply missed a lot of it. My understanding of the tabloids and gossip instead focus on the idiotic and outrageous stuff they do, people they're seen with, feuds (real and otherwise), and the like (and if it's really slow, then maybe one putting out the garbage while wearing a bathrobe, news you need to know

Which is strange now that I think about because they're often given free clothes and jewelry (so ironically some of the few people that can afford it get it for free!) because it presumably gets other people to pay a ridiculous price to get the same thing, so I'd think cosmetic surgeons would be sure to cash in on that as well, even if they had to be underhanded about it.
