Anya has definitely caused more death than any other character. I don't remember the story about making a guy combust, but he may be the only person she has killed directly that we know of. However, if we count every single person that has died as a result of her spells, that would be an inconceivably high number. And even if many of these deaths are somewhat accidental, she is expressly working to cause as much damage as possible.
Willow and Xander kill Cordelia in "The Wish," but it would not have happened if Anya had not cast a spell that lead to Willow and Xander becoming vampires. Of course, that is later undone, but there would be billions other examples like that.
In "Selfless," Anya sets loose a spider that kills several frat boys. Now, the frat boys she clearly meant to kill, but she also leaves the girl she pretended to help inside the house with the spider. When someone makes a wish, it gives the vengeance demon permission to cast a very powerful spell, and if the spell kills the "client" or anyone else, then all the better. Vengeance demons do not have complete control of the consequences of their actions, but they do normally revel in them.
